تعیین میزان بروز و عوامل خطر عفونت زخم در جراحی های تمیز ستون فقرات: یک مطالعه همگروهی آینده¬نگر
The Incidence and Risk Factors of Surgical Wound Infection after Clean Spinal Surgeries: a Prospective Cohort Study
زمینه و هدف: عفونت پس از عمل جراحی ستون فقرات از شایعترین و مهمترین عوارض بعد از عمل تلقی می¬شود. جهت تعیین بهتر عوامل پیش¬بینی کننده خطر رخداد عفونت پس از جراحی نیاز به مطالعات با حجم نمونه بالا و کیفیت مناسب جمع¬آوری داده¬ها می¬باشد.
مواد و روشها: در این مطالعه تعداد 987 بیمار جراحی شده با اتیولوژی¬های متفاوت در بازه زمانی سه ساله (1392-1389) بصورت آینده¬نگر وارد گردیدند.
یافتهها: از این حجـم 27 مورد (7/2%) دچار عفونت بعد از عمل شدند. آزمون¬های آماری چند متغیره نشان دادند که عوامل ذیل با ایجـاد عفـونت ارتبـاط مستقیم دارند: تروما به عنوان علت جراحی، وجود سابقه قبلی دیابت، استعمال دخانیات، در بستر ماندن درطول دوره جراحی، سطح گلوکزسرمی بالاتر از mg/dl120، افزایش طول انسزیون جراحی و افزایش مدت زمان بستری در بیمارستان.
نتیجهگیـری: با توجه به قابل پیشگیری بودن عمده علل فوق، به نظر می¬رسد که می¬توان با اقدامات لازم باعث کاهش قابل توجهی در میزان عفونت گردید.
Introduction & Objective: Post-operative surgical site wound infection (SSWI) is one of the most common complications after spine surgeries. To detect the risk factors for such a complication, studies with large numbers of patients and high quality data-gathering and analysis are required.
Materials & Methods: In this study, we prospectively included 987 patients who underwent operation for different pathologies over a three-year period (2010-2013).
Results: Among these patients, 27 (2.7%) developed infection in the post-operative period. Multi-variant analysis indicated the factors associated with increased risk of post-operative infection as: trauma as the main etiology of damage in trauma cases, past-history of diabetes and smoking, patients being bed ridden in the peri-operative period, blood sugar levels above 120 mg/dl during the treatment period, increased length of incision, and increased hospital stay.
Conclusions: Considering the preventable nature of most of the causes, it seems possible to reduce the chance of infection by controlling these factors.
