بررسی شیوع و شدت یافته های پاتولوژیک در ام آرآی گردن بیماران کاندید جراحی لومبوساکرال به علت هرنیاسیون دیسک، تنگی کانال و اسپوندیلولیستزیس مراجعه کننده به بیمارستان سینا و بررسی اثر این یافته ها بر فرجام کوتاه مدت جراحی بین سال های 1392 تا 1394
Prevalence and Severity of Pathological Findings on Cervical MRI in Patients Candidate Lumbo_Sacral Surgery (Narrow Cervical Canal and Spondylolisthesis and Lumbar Disc Herniation) and Effect Assessment of these Findings on Early Outcome in Patients Admitted to Sina Hospital between 2014 to 2016
زمینه و هدف: تغییرات دژنراتیو ستون فقرات در نتیجه عوامل ژنتیکی و نیز افزایش سن و همچنین عوامل محیطی متنوع در افراد ایجاد می¬شود. در بالین اغلب با بیمارانی مواجه می¬شویم که همزمان بیماری¬های دژنراتیو در نواحی مختلف ستون فقرات دارند و نیز مطالعات بالینی تغییرات دژنراتیو همزمان در ناحیه لومبار و سرویکال را گزارش کرده¬اند. در ارزیابی یافته¬های تصویربرداری تغییرات دژنراتیو ناحیه لومبار با تغییرات دژنراتیو در سایر نواحی ستون فقرات از جمله ناحیه گردن همراهی داشته که می¬تواند منجر به اثرات فشاری بر نخاع و ریشه¬های عصبی شود.
مواد و روشها: این مطالعه به صورت یک مطالعه آینده¬نگر در بین بیماران مراجعه¬کننده به بیمارستان سینا انجام گردید. تعداد جمعیت مورد مطالعه 260 نفر شامل 100 نفر بیمار با اسپوندیلولیستزیس 100 نفر بیمار تنگی کانال کمری و 60 نفر بیمار دچار هرنیاسیون دیسک کمری طی بازه زمانی سال 1392 تا 1394 بوده که معیارهای ورود را داشته و وارد مطالعه گردیدند. برای همه بیماران قبل از عمل و روز سوم پس از عمل پرسشنامه¬های ODI & JOA تکمیل گردید. تمامی بیمارات تحت ام آر آی سرویکال قرار گرفته و نتایج آن توسط یک رادیولوژیست و یک جراح اعصاب تفسیر شده و بیماران بر اساس یافته¬های پیدا شده در ام آر آی و بر اساس مقیاس¬های استاندارد، در دو گروه سرویکال مثبت و سرویکال منفی قرار گرفتند و نتایج آنها در سه گروه و با نرم افزار SPSS شماره نوزده بررسی گردید.
یافتهها: از مجموع 100 بیماری که با پاتولوژی اسپوندیلولیستزیس در ناحیه لومبوساکرال وارد مطالعه شدند، 77 نفر از بیماران خانم و تعداد 23 نفر آقایان بودند. از این تعداد 21 نفر دارای پاتولوژی گردن (گروه سرویکال مثبت) بودند. همچنین در گروه تنگی کانال کمری از مجموع تعداد 100 بیمار وارد شده به مطالعه 72 نفر خانم و 28 نفر آقایان بوده و تعداد بیماران دارای پاتولوژی سرویکال مثبت 24 نفر بود. در گروه دیسک کمری نیز 60 بیمار در مطالعه وارد شده که 36 نفر را خانم¬ها و 24 نفر را آقایان تشکیل می-دادند. شیوع تغییرات پاتولوژیک سرویکال مثبت در بیماران هرنیاسیون دیسک لومبوساکرال 9 نفر بود.
نتیجهگیـری: با توجه به عوامل فوق بهتر است قبل از اعمال جراحی ناحیه لومبوساکرال خصوصاً در افراد با اسپوندیلولیستزیس و تنگی کانال کمری و نیز در افراد مسن جهت بررسی و پیشگیری از حوادث نورولوژیک پس از عمل و نیز رضایت بیشتر بیماران مسن از بهبودی علایم شکایت شده تصویربرداری ناحیه سرویکال نیز انجام گیرد.
Introduction & Objective: Degenerative changes of the spine as a result of genetic factors and aging, as well as environmental factors caused by a variety of people. Often encountered in clinical practice with patients at the same time in different areas of the spine with degenerative diseases as well as degenerative changes in the lumbar and cervical clinical studies have reported.
Materials & Methods: This study, based on a study of prospective cohort of patients admitted to Sina hospital were the total number of patients studied population of 260 people, including 100 people spondylolisthesis 100 patients with lumbar stenosis and 60 patients suffering lumbar disc herniation during the period 2014 to 2016 met the inclusion criteria and were enrolled for all patients preoperative and 3 days postoperative questionnaires ODI & JOA was completed. in all patients undergoing MRI of the cervical and the results it by a radiologist and a neurosurgeon interpreted and patients based on the findings found MRI of the cervical two groups on the basis of standard scales of positive and negative cervical said, and the results were evaluated in three groups.
Results: In patients with spondylolisthesis: The data indicated that variations of both standard JOA score and ODI before and after surgery in patients with negative cervical pathology than patients with positive cervical pathology, the more score improvement percent in cervical patients negative than positive cervical patients. In patients with lumbar stenosis: those with positive and negative pathology of the cervical region, the base of the JOA scores were significantly different from each other after surgery, patients with negative cervical pathology than patients with positive cervical pathology significantly with lower JOA scores were higher ODI data also showed that the improvement percent in patients with negative cervical higher than patients with positive cervical in patients with lumbosacral Disc herniation: In a survey conducted after the surgery, significant differences between the two groups in terms of JOA score and ODI not reported.
Conclusions: Due to the above factors to better pre-operative area of lumbosacral and particularly in patients with spondylolisthesis and lumbar stenosis, and also in older people to study and prevention of postoperative neurological events and more satisfied elderly patients complained of symptoms imaging may also be cervical area performed.
