بررسی اثرات شیمی درمانی در ایجاد پوکی استخوان مردان 60-30 ساله مبتلا به سرطان در مقایسه با گروه کنترل
Assessing Chemotherapy Affect on Osteoporosis in Comparative Study between 30-60 Years old Men with Cancer and Control Group
زمینه و هدف: استئوپروز بیماری شناخته شده¬ای است که بیش از 75 میلیون نفر را در اروپا، ژاپن و آمریکا مبتلا کرده و فقط در اروپا و آمریکا بیش از 3/2 میلیون شکستگی در سال ایجاد می¬کند. پیشگیری از استئوپروز و شکستگی¬های مرتبط با آن اساس سلامت، کیفیت زندگی و استقلال افراد مسن است. لذا این پژوهش با هدف بررسی رابطه کموتراپی و استئوپروز می¬باشد، تا در صورت وجود رابطه معنادار پیش¬بینی¬های لازم به منظور جلوگیری از این عارضه به عمل آید.
مواد و روشها: این مطالعه به صورت مورد - شاهدی و از نوع مقطعی - تحلیلی انجام شده است. تعداد 120 نفر مرد در دو گروه 60 نفری مورد و شاهد که در گروه سنی 60-30 ساله قرار داشتند. این افراد از شهریور ماه سال 1392 تا پایان اسفند ماه 1393 به بیمارستان امام رضا (ع) ارتش مراجعه کردند و حداقل یک سال از اولین شیمی درمانی¬ آنها (در گروه مورد) گذشته بود. گروه شاهد مردان مراجعه¬کننده به درمانگاه داخلی این مرکز در همان زمان بودند. میزان تراکم استخوان هر دو گروه با روش DEXA اندازه-گیری شد. همچنین در دو گروه، مقادیر کلسیم، فسفر، آلکالین فسفاتاز و کنترل شاخص توده بدنی، ثبت و بررسی شدند. اطلاعات پس از جمع¬آوری توسط نرم افزار 19 SPSS تحلیل گردید.
یافتهها: با توجه به نتایج بدست آمده در این مطالعه میانگین شاخص توده بدنی در گروه مورد به میزان قابل توجهی کمتر از گروه شاهد بود (008/0 P =). میانگین میزان تراکم معدنی استخوانی مچ دست در گروه مورد و شاهد اختلاف معنی¬داری را نشان می-داد (013/0 P =) و همچنین میانگین میزان تراکم معدنی استخوان فمور و ستون مهره¬ای در گروه مورد و شاهد اختلاف
معنی¬داری را نشان می¬داد، فمور (007/0 P =) و (ستون مهره¬ای 029/0 P =). استخوان مهره در گروه مورد به میزان قابل توجهی کمتر از گروه شاهد بود (029/0 P =) .
نتیجهگیـری: در مجموع و بر اساس نتایج حاصل از این مطالعه چنین استنباط می¬شود، بیمارانی که به علت سرطان تحت شیمی درمانی قرار می¬گیرند، در معرض خطر قابل توجهی از نظر شکستگی در رابطه با افزایش از دست دادن بافت استخوان هستند، و لازم است این بیماران با اندازه¬گیری منظم میزان تراکم معدنی استخوان برای ارزیابی اثر درمان پیشگیرانه مانیتور شوند.
Introduction & Objective: Osteoporosis is a Known Disease that more than 75 million people in Europe, Japan and North America suffering from it and in Europe and America along with over 2.3 million fractures per year. Prevention of osteoporosis and related fractures is the health basis, quality of life and independence of older people. Therefore, this study examined the relationship between chemotherapy and osteoporosis if there is any significant effect, measures to prevent these complications occurs.
Materials & Methods: In this case – control study, case group was selected among the 60-30 year old patients with cancer admitted to blood unit of Imam Reza Hospital and control group was selected among the patients were admitted to the internal medicine clinic. The bone density for 2 groups was measured using DEXA. Also the serum levels of CA, P, Alk. P were measured among both case and control groups.
Results: 120 patients were enrolled in two groups of 60 persons contained case and control. Patient age ranged from 30 to 60 years. The average age of the people who participated in the study was 48.05 ± 7.99 years. This rate in the treatment group and the control group was 47.96 ± 8.20 years was 48.15 ± 7.84 years. The mean BMI was significantly lower in the case group then the control group (P-value = 0.008). The mean of serum blood calcium, phosphorus and alkaline phosphates no statistically significant difference between the two groups. Average wrist bone mineral density in case group was -2.85± 1.13 and
-2.29 ± 1.29 in controls. Wrist bone mineral density was significantly lower in cases then controls
(P-value = 0.007). The average amount of vertebral bone mineral density in patients was – 1.87 ± 1.27 and -1.37± 1.31 in controls. Spine bone mineral density was significantly lower in the case group then the control group (P-value = 0.039).
Conclusions: We believe that patients who receive chemotherapy for cancer are at increased risk of bone loss. These patients should be evaluated by measuring BMD regularly monitored to assess the effectiveness of preventive treatment. Taking special measures for early detection and prevention of chemotherapy – induced osteoporosis can significantly reduce the Morbidity and Morbidity in this field and improved the quality of life of patients who are undergoing chemotherapy.
