بررسی میزان فراوانی عفونت هلیکوباکترپیلوری در نمونه¬های پاتولوژی و انکوژن HER2/neu در بیماران مبتلا به سرطان معده و ارتباط آن با مرحله بیماری در زمان تشخیص در مرکز درمانی شهدای تجریش
Evaluation of Incidence of Helicobacter Pylori infection and HER2/neu Oncogene Expression in Pathology Samples of Patients with Gastric Cancer and their Association with Stage of Disease in Shohada Tajrish Hospital
زمینه و هدف: سرطان معده یکی از شایعترین سرطان¬ها در جهان است که علیرغم پیشرفت¬های صورت گرفته در زمینه درمان، میزان بقای آن پایین است که از علل مهم آن مراجعه دیر جهت درمان است. هلیکوباکترپیلوری و بیان بیش از حد ژن HER2 به عنوان فاکتورهای پروگنوستیک سرطان معده مطرح شده¬اند. در این مطالعه ما ارتباط این دو فاکتور را در سرطان معده مورد بررسی قرار داده¬ایم.
مواد و روشها: در این مطالعه گذشته¬نگر 59 نفر از بیماران مبتلا به سرطان معده مراجعه کننده به بیمارستان شهدای تجریش طی سال¬های 1393 و 1394 که تحت عمل جراحی گاسترکتومی قرار گرفته بودند، مورد بررسی واقع شدند. بررسی هیستوپاتولوژیک نمونه-های برداشته شده، انجام شد و میزان فراوانی موارد مثبت از نظر هلیکوباکترپیلوری و Her2/neu تعیین شد. مرحله بیماری هم مشخص گردید. داده¬ها با نرم افزار SPSS نسخه 20 آنالیز شدند.
یافتهها: 59 بیمار مورد بررسی قرار گرفتند (میانگین سنی 9/12 ± 0/60 سال، مردان 6/74%). از بین بیماران 4/42% مثبت از جهت هلیکوباکترپیلوری بودند. وضعیت Her2/neu به صورت 6/18 درصد 3+، 5/8 درصد 2+، 9/11درصد 1+ و 0/61 درصد منفی بود. فراوان¬ترین مرحله، مرحله IV بود (5/47%). مثبت شدن HER2/neu با مرحله بالاتر از بیماری و بقای کمتر ارتباط داشت
(P-value ها هر دو 015/0). ارتباط مثبـت بودن هلیکـوباکتر پیلـوری با مثبـت بودن HER2/neu مـورد سنجش واقع شد که معنادار نبود (104/0 = P -value).
نتیجهگیـری: در مجموع در مطالعه ما نکته مهم ارتباط بیان HER2/neu با بقای کوتاه¬تر است و سایر ارتباطات معنادار به دست نیامده است. مثبت بودن هلیکوباکترپیلوری نمی¬تواند مثبت شدن HER2/neu را پیش بینی کند.
Introduction & Objective: Gastric cancer is one the most prevalent cancers around the world and is associated with low survival rates despite the advances in therapy modalities. This low rate of survival is mainly due to delayed seeking for treatment. Helicobacter pylori and HER2/neu gene overexpression are suggested as prognostic factors for gastric cancer. In this study, we evaluated the association of these two factors in gastric cancer.
Materials & Methods: Fifty-nine patients with gastric cancer who had undergone gastrectomy in Shohda-Tajrish hospital during 2014 to 2015 were evaluated in this study. Histopathologic assessment of the samples was done and the frequencies of Helicobacter pylori positivity and HER2/neu overexpression were measured. The stage of gastric cancer was determined in patients. The data were analyzed with SPSS 20.0 and the associations of study parameters were investigated.
Results: Mean age was 60.0 years. 74.6% of the study patients were male. 42.4% were positive for Helicobacter pylori. The status of HER2/neu membrane staining was as follows: 3+ (18.6%), 2+ (8.5%), 1+ (11.9%) and negative (61%). The most common stage was stage IV (47.5%). The association between Helicobacter pylori and HER2/neu expression was not significant (P > 0.05). The HER2/neu overexpression was correlated with disease stage and survival duration (P < 0.05).
Conclusions: Overall, there was a potential correlation between HER2/neu overexpression and lower survival in our study. There was no other significant association between parameters. Positive Helobacter pylori could not predict HER2/neu overexpression.
