بررسی شیوع و عوامل خطر فتق انسزیونال به دنبال عمل جراحی لاپاروتومی بعد از گذشت یک سال در بیمارستان دکتر شریعتی اصفهان در سال 1399
Evaluation the Prevalence and Risk Factors of Incisional Hernia following Laparotomy Surgery after one Year follow up in Esfahan Shariati Hospital in 2020
زمینه و هدف: فتق برشی یا اینسزیونال یکی از شایع¬ترین عوارض بعد از عمل جراحی شکم است. این فتق بعلت عدم ترمیم فاشیای عضلات جدار شکم و جدا شدن فاشیای عمقی از همدیگر در محل عمل جراحی پیشین بوجود می¬آید. بنابراین، هدف از مطالعه حاضر بررسی شیوع و عوامل خطر فتق اینسزیونال به¬دنبال عمل جراحی لاپاراتومی بود.
مواد و روشها: معیارهای ورود به مطالعه شامل کلیه بیماران تحت جراحی لاپاراتومی، گذشت یک سال از عمل جراحی لاپاراتومی¬شان و تمایل به شرکت در مطالعه بود. عدم تمایل به شرکت در مطالعه به¬عنوان معیار خروج در نظر گرفته شد. سیصد و شصت بیمار دارای معیارهای ورود که از تاریخ 28/1/1399 تا تاریخ 30/6/1399 تحت جراحی لاپاراتومی قرار گرفته بودند به روش نمونه-گیری در دسترس وارد مطالعه شدند. داده¬ها با استفاده از پرسشنامه دموگرافیک و معاینه فیزیکی جمع¬آوری شدند.
یافتهها: نتایج پژوهش حاضر نشان داد که میزان بروز فتق اینسزیونال 2/7% بود. فراوانی بروز فتق اینسزیونال به دنبال عمل جراحی لاپارروتومی بر اساس سن، جنس، ابتلا به فشار خون، دیس لیپدمی، بیماری ایسکمیک قلبی، نوع جراحی، نوع نخ بخیه، نوع برش جراحی، مصرف سیگار، اعتیاد و مدت بستری تفاوت آماری معناداری نداشت. اما مشخص شد که فراوانی بروز فتق اینسزیونال به دنبال عمل جراحی لاپاراتومی در بیماران دارای اضافه وزن و چاق بیشتر از بیماران با شاخص توده بدنی نرمال (2/12% در مقابل 2/2%)، در بیماران دارای دیابت به صورت معناداری بیشتر از بیماران بدون دیابت (25% در مقابل 5/4%) و در بیماران دارای عفونت محل جراحی به صورت معناداری بیشتر از بیماران بدون عفونت محل جراحی (5/37% در مقابل 3/4%) بود (P < 0.001).
نتیجهگیـری: با توجه به ارتباط مثبت چاقی و عوارض پس از جراحی در بروز فتق اینسزیونال، پیشنهاد می¬شود تمام بیماران دارای ریسک فاکتور و نیازمند جراحی الکتیو، قبل از جراحی میزان شاخص توده بدنی خود را کاهش داده و برای تمام بیماران نیازمند جراحی (اورژانسی و الکتیو) از بروز عفونت محل جراحی جلوگیری کرده تا بروز فتق اینسیزیونال کاهش یابد.
Introduction & Objective: Incisional hernia is one of the most common complications after abdominal surgery. This hernia occurs due to failure to repair the fascia of the abdominal wall muscles and the separation of the deep fascia from each other at the site of previous surgery. Therefore, the aim of this study was to investigate the prevalence and risk factors of incisional hernia following laparotomy after one year.
Materials & Methods: Inclusion criteria included all patients undergoing laparotomy, one year after their laparotomy, and willingness to participate in the study. Reluctance to participate in the study was considered as an exclusion criteria. Three hundred and sixty patients with inclusion criteria who undergoing laparotomy from April/16/2020, to September/20/2020, were included in the study by convenience sampling method. Data were collected using a demographic questionnaire and physical examination.
Results: The results of current study showed that the incidence of incisional hernia was 7.2%. The incidence of incisional hernia following laparotomy was not significantly different based on age, sex, hypertension, dyslipidemia, ischemic heart disease, type of surgery, suture type, incision type, smoking, addiction and length of hospital stay. However, it was found that the frequency of incisional hernia after laparotomy in overweight and obese patients was significantly higher than patients with normal BMI (12.2% vs. 2.2%), in patients with diabetes was significantly higher than patients without diabetes
(25% vs. 4.5%), and in patients with infection at the surgical site was significantly higher than patients without infection site of surgery (37.5% vs. 4.3%) (P < 0.001).
Conclusions: Considering the positive relationship between obesity and postoperative complications in incidence of Incisional hernia, it is recommended to reduce their BMI before surgery for all patients with risk factors and in need of elective surgery, and for all patients in need of surgery (emergency and Elective) prevents infection at the surgical site to reduce the incidence of Incisional hernia.
