Article abstract: Congenital diaphragmatic hernia is caused because of abnormal formation of muscular parts of diaphragm. The incidence of CDH in common births is ranged from 1:2000 to 1:5000. Pulmonary hypoplasia and pulmonary hypertension are factors that increase mortality and morbidity of CDH. We reported a 68 years Old Iranian woman with pain and tenderness in right upper quadrant area because of CDH. This hernia first diagnosed with CXR and then chest and abdomen sonography and high resolution CT scan and venous-oral CT scan without contrast. In images we observed a defect in posterolateral right diaphragm with Omentum hernia and transverse colon inside it, and we used right posterolateral thoracotomy to cure this defect. In this case CT and CXR was very useful for diagnosing CDH, in spite of all that CDH diagnosis before operation is too difficult because of rare cases and different types of CDH. Also In Order To Improve Clinical Cares in Adult CDH patients, it is recommended to investigate more cases and following them in long term periods.
