Abstract
Primary intrathoracic goiter, especially of posterior mediastinum were rare and were less than 1%. Primary intrathoracic goiters supplied by intrathoracic blood vessels. We introduce a patient who referred with a huge mass in the posterior mediastinum extended to the anterior superior mediastinum with a sign of dyspnea and chronic cough. This mass was easily removed through standard postrolateral thoracotomy and all the symptoms of the patient were resolved .The blood supply of mass arises from intrathoracic vessels.
