ارتباط هیپرتروفی تنه عصبی در نمونه بیوپسی رکتوم با طول روده آگانگلیونیک در کودکان مبتلا به بیماری هیرشپرونگ مراجعه کننده به مرکز طبی کودکان تهران طی سال های ۱۳۹۰ الی ۱۳۹۶

چکیده
زمینه و هدف: بیماری هیرشپرونگ یک بیماری حرکتی دستگاه گوارش هست که به دلیل نقص در مهاجرت سلول‌های عصبی در طول تکامل دستگاه گـوارش ایجاد می‌شود که سبب انسداد عملکـردی می‌گردد. تشخیص این بیماری با بیوپسی رکتوم و فقـدان گانگلیـون و هیپرتروفی تنه عصبی در قطعه درگیر می‌باشد. مطالعه حاضر به منظور بررسی ارتباط هیپرتروفی تنه عصبی در نمونه بیوپسی با طول روده فاقد گانگلیون انجام شد. مواد و روش‌ها: این مطالعه مقطعی، برروی بیماران هیرشپرونگ که به مرکز طبی کودکان تهران طی سال‌های 1390 الی 1396 مراجعه کرده‌اند، انجام گرفته است. خصوصیات دموگرافیک بیماران از جمله سن، جنس، نوع عمل جراحی، طول قطعه درگیر طی جراحی ثبت شده، لام پاتولوژی بیماران از نظر اندازه قطر و تعداد تنه عصبی ثبت شده و ارتباط بین قطر تنه عصبی در قطعه فاقد گانگلیون با طول روده فاقد گانگلیون مورد بررسی قرار گرفت. برای داده‌های تحلیلی از آزمون کای اسکویر و آزمون دقیق فیشر استفاده شد. یافته‌ها: تعداد ۲۵ کودک مبتلا به بیماری هیرشپرونگ برحسب جنس و سن مورد بررسی قرار گرفتند، پسرها بیشترین فراوانی را داشتند و عمده بیماران (۷ -28% موارد) را نوزادان (تا یک ماه) تشکیل می‌دهند. بیشترین محل درگیری طبق انتظار رکتوسیگمویید بود. از تعداد ۲۵ بیمار ۶ بیمار دارای تنه عصبی کمتر از ۴۰ میکرومتر بودند که از این تعداد فقط ۲ بیمار درگیری کل کولون داشتند. هیچ ارتباطی بین قطر تنه عصبی و سن و جنس و طول روده درگیر یافت نشد. نتیجه‌گیـری: این فرضیه که میزان هیپرتروفیه شدن تنه عصبی می‌تواند با درگیری وسیع‌تر روده و بالاتر بودن منطقه انتقالی (Transition Zone) همراه باشد، تایید نشد، همچنین ارتباطی بین تعداد تنه عصبی موجود در نمونه بیوپسی رکتوم و طول درگیری نیز یافـت نشد. با این حال همچنان با توجه به نتایج می‌توان گفـت که در بیماری هیرشپرونگ هیپـرتروفی شدن تنه عصبی در نمونه پاتولوژی در قطعه آگانگلیونی دیده می‌شود.
