تغییرات پروفایل آهن پس از جراحی باریاتریک در بیماران مراجعه کننده به کلینیک چاقی در بیمارستان حضرت فاطمه الزهرا(س)

چکیده
زمینه و هدف: چاقی به عنوان یک بحران جهانی، علاوه بر ایجاد بار اقتصادی، به افزایش شیوع بیماری‌های مرتبط منجر شده است. جراحی باریاتریک به عنوان مؤثرترین روش درمانی برای کاهش وزن در افراد چاق، با عوارض بالقوه‌ای از جمله کمبود تغذیه‌ای همراه است. هدف این مطالعه بررسی تغییرات 6 ماهه پروفایل آهن پس از جراحی باریاتریک در بیماران مبتلا به چاقی مراجعه‌کننده به کلینیک چاقی بیمارستان حضرت فاطمه الزهرا(س) بود. مواد و روش‌ها: این مطالعه به صورت کوهورت گذشته‌نگر، بر روی 146 بیمار چاق کاندیدای جراحی باریاتریک در بازه زمانی مهر 1402 تا مهر 1403 انجام شد. بیماران فاقد بیماری‌های مرتبط با کم‌خونی و بدون مصرف مکمل‌های آهن یا داروهای تأثیرگذار بر متابولیسم آهن وارد مطالعه شدند. اطلاعات دموگرافیک، آنتروپومتریک، و آزمایشگاهی شامل شاخص‌های پروفایل آهن (فریتین، آهن سرم، هموگلوبین و ...) از دیتابیس ملی جراحی چاقی استخراج و در زمان‌های قبل از جراحی، سه و شش ماه پس از جراحی ثبت شد. داده‌ها با استفاده از آزمون‌های آماری توصیفی و نیز تحلیلی مانند آزمون‌های آنالیز واریانس یک طرفه، تحلیل اندازه‌های تکراری (Repeated Measure Anova) در نرم‌افزار SPSS نسخه 25 تحلیل شدند. مقدار احتمال کمتر از 05/0 به عنوان سطح معنا دار در نظر گرفته شد. یافته‌ها: در گروه بیماران تحت مینی‌بای‌پس تک آناستوموزی، تغییرات فریتین در طول زمان از نظر آماری معنادار بود. میانگین سطح فریتین در این روش از 84/72±55/68 نانوگرم بر میلی‌لیتر پیش از جراحی، به 41/73±75/87 در سه ماه 69±45/25 در شش ماه پس از جراحی افـزایش یافت. در سایر گـروه‌های جراحی (بای‌پس معـده Roux-en-Y، گاستـرکتومی اسلیو و بای‌پس ایلئـوگاستریک) تغییرات فریتین طی زمان از نظر آماری معنادار نبـودند. اثر زمان بر سطح ویتامین ب 12 معنادار بود (03/0 = مقدار احتمال). بطوریکه میانگین سطح ویتامین ب 12 برای تمامی گروه‌ها در زمان های مختلف افزایش بافته بود. همچنین برهم‌کنش بین زمان و نوع جراحی معنادار گزارش شد (004/0 = مقدار احتمال). مقایسه‌های جفتی نشان دادند که در گروه بیماران تحت مینی‌بای‌پس تک آناستوموزی، افزایش سطح ویتامین ب 12 از زمان پیش از جراحی تا سه ماه و 6 ماه پس از آن ونیز افزایش آن از سه تا شش ماه بعد نیز معنادار بود (02/0 = مقدار احتمال). همچنین در گروه گاسترکتومی اسلیو نیز افزایش سطح ویتامین ب 12 از پیش از جراحی تا سه ماه و 6 ماه پس از آن معنادار بود (02/0 = مقدار احتمال). نتیجه‌گیـری: جراحی باریاتریک تأثیر قابل توجهی بر پروفایل آهن بیماران دارد و این تغییرات بسته به سن و جنسیت متفاوت است. نتایج مطالعه حاضر بر اهمیت پایش و مداخلات تغذیه‌ای پس از جراحی چاقی برای پیشگیری از کمبودهای تغذیه‌ای تأکید دارد. مطالعات بیشتر با حجم نمونه بزرگ‌تر و پیگیری طولانی‌مدت توصیه می‌شود.
