بررسی ویژگی‌های بالینی و جمعیت ‌شناختی بیماران قرار گرفته تحت عمل جراحی تیروئید در جمعیت جنوب ایران بر اساس برنامه ثبت ملی جراحی تیروئید

چکیده
زمینه و هدف: بدخیمی‌های تیروئید از شایع‌ترین انواع بدخیمی به حساب می‌آیند که بروز آن‌ها طی سال‌های اخیر روند افزایشی داشته است. این بیماری خود را به صورت ضایعات نودولار نشان می‌دهد. جراحی تیروئید به عنوان درمان استاندارد در بیماران مبتلا به سرطان غده‌ تیروئید محسوب می‌گردد. در جمعیت جنوب ایران نیز این بدخیمی شایع است و عمده‌ بدخیمی‌های مربـوط به غدد درون‌ریز را شامـل می‌شود. بر همیـن اساس، در این مطالعـه به بررسی ویژگـی‌های بالینـی و جمعیت ‌شناختی بیماران قرار گرفته تحت عمل جراحی تیروئید در جمعیت جنوب ایران پرداخته شده است. مواد و روش‌ها: در این مطالعهی توصیفی - مقطعی، اطلاعات بیماران از بانک اطلاعاتی برنامه‌ی ملی ثبت جراحی تیروئید ایران استخراج شده و مورد ارزیابی قرار گرفته است. تمام بیمارانی که طی سال‌های 1402 و 1403 تحت عمل جراحی تیروئید قرار گرفته‌اند و مشمول معیارهای خروج از مطالعه نبودند، وارد مطالعه شده‌اند. داده‌های جمعیت ‌شناختی، سوابق بیماری، داده‌های آزمایشگاهی و داده‌های مربوط به عمل جراحی پس از عمل بیماران استخراج و بررسـی شده است. برای داده‌هـای کمی پیوسته، میانگین و انحراف از معیار و برای داده‌های کمی گسسته، فراوانی و درصد فراوانی گزارش گردیده است. یافته‌ها: تعداد 1409 بیمار وارد مطالعه شدند که 274 (4/19%) آن‌ها را مردان و 1135 (6/80%) آن‌ها را زنان تشکیل می‌دادند. میانگین سنی جمعیت 96/12±33/44 سال بود. بیشترین جراحی انجام گرفته، تیروئیدکتومی توتال بود ]1307 بیمار (8/92 %)[. شایع‌ترین یافته در پاتولوژی در پس از عمل را سرطان سلول‌های پاپیلاری تیروئید[Papillary Thyroid Carcinoma (PTC)] (2/32%) تشکیل می‌داد؛ پس از آن MNG (7/19%)، نوع فولیکولار PTC (7/17%)، PTMC (1/15%)، هایپرپلازی خوش‌خیم نودولی (7/14%)، هاشیموتو (6/2%)، آدنوم پاراتیروئید (4/2%)، بیماری گریوز (8/1%)، بدخیمی سلول‌های هرتل (4/1%) و MTC (7/0%) شایع‌ترین بودند. همچنین، شایع‌ترین یافته‌ها در آسپیراسیون با سوزن ظریف[Fine Needle Aspiration (FNA)] پیش از عمل را به ترتیب PTC (9/43%)، یافته‌های خوش خیم (1/17%)، AUS/FLUS (9/7%)، MNG (9/7%)، بیماری گریوز (3/2%) تشکیل می‌دادند. نتیجه‌گیـری: در این مطالعه که اطلاعات بیمارانی که تحت عمل جراحی تیروئید قرار گرفتند بررسی شده است، دیده شد که زنان بیشتر از مردان کاندید جراحی تیروئید بودند. همچنین سرطان سلول‌های پاپیلاری تیروئید شایع‌ترین نوع بدخیمی در بیماران بود.
