بررسی میزان عود و بقای بیماران مبتلا به سرطان پستان پیشرفته موضعی پس از ماستکتومی رادیکال اصلاح شده و بازسازی با استفاده از فلپ شکم

چکیده
زمینه و هدف: در حال حاضر ماستکتومی رادیکال اصلاح شده با بازسازی با کمک فلپ‌های شکمی به عنوان یک تکنیک جراحی رایج قابل قبول در درمان سرطان پستان پیشرفته موضعی مطرح است. با این حال اطلاعات مربوط به میزان عوارض و موفقیت آن در مطالعات قبلی دارای تناقض‌های متعددی می‌باشد. لذا، این مطالعه با هدف بررسی میزان عوارض، عود و بقای بیماران مبتلا به سرطان پستان موضعی و بازسازی به روش فلپ شکم انجام شد. مواد و روش‌ها: در این مطالعه مقطعـی بیماران مبتلا به سرطان پستان موضعی پیشرفته که در بیمارستان شهدای تجریش تهران طی سال‌های 1391 تا 1392 به روش ماستکتـومی رادیکال اصلاح شده همراه با بازسازی پستان با کمک فلپ شکمی درمان شده بودند، انجـام شد. در ابتدا بر حسب معیـارهای ورود شامل: نوع هیستوپاتولوژی سرطان پستان Invasive Ductal Carcinoma باشد، تمـام بیماران توسط یک جراح درمان شـده باشند، مرحله بیماری از نوع پیشرفته، و فقدان متاستاز دوردست، بیماران انتخاب شده و سپس میزان رضایتمندی، بروز عـوارض و میـزان بروز عود و همچنین بقا به کمک معاینات دوره‌ای سه ماهه و به مـدت 48 ماه ثبت شد. یافته‌ها: در مطالعه حاضر میانگین سنی بیماران 7/55 سال (با انحراف معیار 2/11 سال) و میزان رضایتمندی از جراحی از نمره VAS معادل 2/1±5/7 در 6 ماه اول به نمره 10 در پایان یک سال رسید. همچنین عود موضعی در هیچ کدام از بیماران رویت نشد. میزان بقای کلی یک ساله 100 درصد بود. با این حال در پایان 48 ماه پیگیری 32 بیمار زنده مانده بودند که نشان دهنده بقای کلی مطلوب (94) درصدی می‌باشد. نتیجه‌گیـری: نتـایج حـاکی از رضـایت بالای بیمـار، میـزان کم عوارض جراحی، عدم عود موضعی و نرخ بقای امیدوارکننده بود.
