فراوانی و عوامل موثر بر اختلالات اسکلتی - عضلانی اندام فوقانی ناشی از پاکلیتاکسل در زنان مبتلا به سرطان پستان جراحی شده: یک مطالعه توصیفی مقطعی ثانویه

چکیده
زمینه و هدف: سرطان پستان شایع‌ترین بدخیمی زنان است و با وجود پیشرفت‌های جراحی، رادیوتراپی و شیمی‌درمانی، عوارض درمانی همچون نوروپاتی محیطی ناشی از پاکلیتاکسل کیفیت زندگی بیماران را تهدید می‌کند. این دارو با اختلال در دینامیک میکروتوبول‌ها و انتقال آکسونی، موجب آسیب عصبی و محدودیت عملکرد اندام فوقانی می‌شود. ترکیب اثرات جراحی، رادیوتراپی و پاکلیتاکسل خطر این مشکلات را تشدید می‌کند و شناسایی فراوانی و عوامل مؤثر آن برای برنامه‌ریزی بالینی ضروری است؛ لذا مطالعه حاضر با هدف فراوانی و عوامل موثر بر اختلالات اسکلتی - عضلانی اندام فوقانی ناشی از پاکلیتاکسل در زنان مبتلا به سرطان پستان جراحی شده انجام شد.
مواد و روش‌ها: در این مطالعه توصیفی – مقطعی که در دانشگاه علوم پزشکی تبریز انجام شد، زنان بالای ۱۸ سال مبتلا به سرطان پستان پس از جراحی و کاندید دریافت پاکلیتاکسل و رادیوتراپی وارد پژوهش شدند. نمونه‌گیری به روش در دسترس بر اساس محاسبه حجم نمونه برای برآورد یک نسبت انجام و ۱۰۰ بیمار وارد شدند. داده‌های دموگرافیک و بالینی جمع‌آوری، و مشکلات اسکلتی – عضلانی با مجموعه‌ای از آزمون‌های فیزیکی استاندارد برای شانه، آرنج، سندرم تونل کارپ و التهاب غلاف تاندون شست بررسی و ثبت گردید. بیماران بر اساس سمت درگیری سرطان پستان به دو سمت درگیر و سمت سالم تقسیم شدند و ابتلا به اختلالات اسکلتی عضلانی در هر سمت بررسی شد. از آزمون­های آماری من ویتنی یو، کای اسکوئر و تی تست نیز برای بررسی نتایج داده­های کیفی استفاده شد. میزان P کمتر از 05/0 در تمامی موارد معنادار تلقی شد.
یافته‌ها: نتایج آزمون‌های فیزیکی شانه نشان داد که بیشترین فراوانی نتایج منفی مربوط به تست Cross ‑ over با 69% (69 نفر) و Yergason با 66% (66 نفر) بود. در تست Empty Can، 21% (21 نفر) پاسخ مثبت در سمت درگیر و 8% (8 نفر) پاسخ مثبت دوطـرفه داشتند. تست Neer Impingement در 23% (23 نفر) در سمت درگیر و 12% (12 نفر) به‌صورت دوطرفه مثبت بود. در تست Hawkins–Kennedy، نتایج مثبت در سمت درگیر در 20% (20 نفر) و دوطرفه در 9% (9 نفر) مشاهده شد. همچنین تست Compression در 25% (25 نفر) در سمت درگیر و تست Cross ‑ over در 2% (2 نفر) به‌صورت دوطرفه مثبت گزارش شد که نشان‌دهنده الگوهای متفاوت درگیری ساختارهای شانه در بیماران می‌باشد.
نتیجه‌گیـری: بر اساس یافته‌های این مطالعه، اختلالات اسکلتی ‌ عضلانی و پاتولوژی‌های شانه در زنان مبتلا به سرطان پستان جراحی شده و دریافت‌کننده پاکلیتاکسل، بیشتر در سمت درگیر بیماری مشاهده شد و شایع‌ترین الگو مربوط به برخورد زیرآکرومیال بود، در حالی‌که آسیب مفصل آکرومیوکلاویکولار کمترین سهم را داشت. این نتایج نشان می‌دهد که ترکیب اثرات جراحی، نوروپاتی دارویی و پیامدهای پرتودرمانی نقش اصلی در بروز این اختلالات دارد و ارزیابی و توان‌بخشی هدفمند ضروری است.
