شبیه سازی بیومکانیک نتایج جراحی آئورتوپلاستی کاهشی در بیماران کاندید عمل تعویض دریچه آئورت: بررسی موارد مراجعه به بیمارستان حضرت رسول اکرم (ص) در سال­های 1402-1403

چکیده
زمینه و هدف: جراحی‌های آئورت به‌عنوان یکی از چالش‌برانگیزترین و حیاتی‌ترین روش‌های جراحی در حوزه قلب و عروق شناخته می‌شوند. آئورتوپلاستی کاهشی یکی از روش‌های جراحی است که به‌عنوان جایگزینی برای پیوند آئورت صعودی در بیماران با اتساع متوسط آئورت استفاده می‌شود. این مطالعه به بررسی تأثیر این جراحی بر همودینامیک بیماران و مقایسه نتایج بالینی با پیش‌بینی‌های شبیه‌سازی عددی می‌پردازد.
مواد و روش‌ها: در این مطالعه، اطلاعات ۴ بیمار که تحت عمل جراحی آئورت قرار گرفتند، جمع‌آوری و تحلیل شد. فشار خون بیماران قبل و بعد از عمل اندازه‌گیری شد و نتایج اکوکاردیوگرافی و شبیه‌سازی‌های اجزای محدود (FEM) و مدل‌سازی عددی برای ارزیابی همودینامیک استفاده شد. داده‌های رادیولوژیک بیماران با استفاده از نرم‌افزار Abaqus و روش اجزای محدود (FEM) تحلیل شدند.
یافته‌ها: کیس اول (مرد ۵۶ ساله) فشار خون بعد از عمل تغییر چندانی نکرد و پیش‌بینی‌های شبیه‌سازی عددی با نتایج اکو تطابق داشت. کیس دوم (مرد ۶۲ ساله) کاهش قابل توجهی در فشار خون بعد از عمل مشاهده شد و پیش‌بینی‌های مدل محاسباتی با نتایج اکو مطابقت داشت. کیس سوم (زن ۷۵ ساله) نارسایی دریچه آئورت پس از عمل به وجود آمد که با پیش‌بینی‌های مدل عددی مطابقت نداشت. کیس چهارم (مرد ۷۰ ساله) کاهش قابل توجهی در فشار خون بعد از عمل مشاهده شد و پیش‌بینی‌های شبیه‌سازی با نتایج اکو تطابق داشت.
نتیجه‌گیـری: نتایج این مطالعه نشان داد که شبیه‌سازی‌های اجزای محدود (FEM) و مدل‌ سازی عددی ابزارهای مفیدی برای ارزیابی نتایج جراحی هستند. با این حال، برای دستیابی به نتایج بهتر و دقیق‌تر، لازم است که محدودیت‌ها و چالش‌های موجود مورد توجه قرار گیرند. توسعه مدل‌های عددی دقیق‌تر با لحاظ کردن ویژگی‌های فردی بیماران، آموزش پزشکان در زمینه استفاده از ابزارهای نوین و انجام پژوهش‌های بیشتر، می‌تواند به بهبود نتایج بالینی بیماران کمک کند.
