بررسی میزان فراوانی عفونت هلیکوباکترپیلوری در نمونه‌های پاتولوژی و انکوژن HER2/neu در بیماران مبتلا به سرطان معده و ارتباط آن با مرحله بیماری در زمان تشخیص در مرکز درمانی شهدای تجریش

چکیده
زمینه و هدف: سرطان معده یکی از شایعترین سرطان‌ها در جهان است که علیرغم پیشرفت‌های صورت گرفته در زمینه درمان، میزان بقای آن پایین است که از علل مهم آن مراجعه دیر جهت درمان است. هلیکوباکترپیلوری و بیان بیش از حد ژن HER2 به عنوان فاکتورهای پروگنوستیک سرطان معده مطرح شده‌اند. در این مطالعه ما ارتباط این دو فاکتور را در سرطان معده مورد بررسی قرار داده‌ایم. مواد و روش‌ها: در این مطالعه گذشته‌نگر 59 نفر از بیماران مبتلا به سرطان معده مراجعه کننده به بیمارستان شهدای تجریش طی سال‌های 1393 و 1394 که تحت عمل جراحی گاسترکتومی قرار گرفته بودند، مورد بررسی واقع شدند. بررسی هیستوپاتولوژیک نمونه-های برداشته شده، انجام شد و میزان فراوانی موارد مثبت از نظر هلیکوباکترپیلوری و Her2/neu تعیین شد. مرحله بیماری هم مشخص گردید. داده‌ها با نرم افزار SPSS نسخه 20 آنالیز شدند. یافته‌ها: 59 بیمار مورد بررسی قرار گرفتند (میانگین سنی 9/12 ± 0/60 سال، مردان 6/74%). از بین بیماران 4/42% مثبت از جهت هلیکوباکترپیلوری بودند. وضعیت Her2/neu به صورت 6/18 درصد 3+، 5/8 درصد 2+، 9/11درصد 1+ و 0/61 درصد منفی بود. فراوان‌ترین مرحله، مرحله IV بود (5/47%). مثبت شدن HER2/neu با مرحله بالاتر از بیماری و بقای کمتر ارتباط داشت (P-value ها هر دو 015/0). ارتباط مثبـت بودن هلیکـوباکتر پیلـوری با مثبـت بودن HER2/neu مـورد سنجش واقع شد که معنادار نبود (104/0 = P -value). نتیجه‌گیـری: در مجموع در مطالعه ما نکته مهم ارتباط بیان HER2/neu با بقای کوتاه‌تر است و سایر ارتباطات معنادار به دست نیامده است. مثبت بودن هلیکوباکترپیلوری نمی‌تواند مثبت شدن HER2/neu را پیش بینی کند.
