اثربخشی متیل پردنیزولون در مقایسه با سایر داروها برای کنترل درد بعد از جراحی دندان مولر سوم: یک مطالعه متاآنالیز

چکیده
زمینه و هدف: درد پس از جراحی دندان مولر سوم منجر به اثرات منفی و نامطلوبی بر ادامه درمان و همچنین نیازهای تغذیه‌ای این بیماران دارد، در چندین مطالعه به اثرات مثبت متیل پریدنیزولون در مدیریت مطلوب درد پس از جراحی دندان مولر سوم اشاره شده است؛ اما هیچ مقایسه‌ای با سایر داروها انجام نشده است؛ لذا تصمیم‌گیری در رابطه با اینکه کدام دارو اثربخشی بیشتری در مدیریت درد پس از جراحی دارد، سخت می‌باشد. لذا مطالعه حاضر با هدف اثربخشی متیل پردنیزولون در مقایسه با سایر داروها برای کنترل درد بعد از جراحی دندان مولر سوم انجام شده است. مواد و روش‌ها: مطالعه حاضر از نوع متاآنالیز بود؛ پایگاه‌های اطلاعاتی PubMed،EMBASE و Cochrane Library، Web of science, Scopus, Mag-Iran, و SID برای شناسایی کارآزمایی‌های بالینی مرتبط با هدف مطالعه حاضر مورد استفاده قرار گرفتند. کلید واژه‌های مورد استفاده عبارت بودند از: متیل پردنیزولون، استامینوفن، تنوکسیکام، کتوپروفن، فلوربیپروفن، دیکلوفناک، ایبوپروفن، داروهای ضد التهـابی غیر استروئیـدی، پردنیزولـون، دگزامتازون، کورتیکواستروئیدها، جراحی دندان مولر سوم، درد پس از عمل، تورم صورت، التهاب، باز شدن دهان، عوارض نامطلوب و پرونده ایمنی. داده‌ها با روش آماری واریانس معکوس، تفاوت میانگین و اثرات ثابت با استفاده از نرم افزار Review Manager 5.3 مورد تجزیه و تحلیل قرار گرفتند. یافته‌ها: ارزیابی درد پس از عمل پس از برداشتن مولر سوم با استفاده از داده‌های دو کارآزمایی بالینی (113 نفر) انجام شد. تجزیه و تحلیل تلفیقی تفاوت آماری را هنگام مقایسه متیل پردنیزولون و سایر داروها در طی 6 روز بعد از عمل نشان نداد (312- =/ RR01/0- تا 522- :CI95% - 12%=I2 و 004/0=P). نتیجه‌گیـری: درد پس از جراحی دندان مولر سوم بسیار مهم می‌باشد و عدم مدیریت آن می‌تواند بر کیفیت زندگی بیماران اثرات نامنفی بر جای بگذارد؛ اتخاذ اقدامات پیشگیرانه می‌تواند در مدیریت این درد مؤثر واقع شوند؛ ارزابی کیفی نشانگر برتری متیل پریدنیزولون برای کنترل درد پس از جراحی دندان مولر سوم نسبت به سایر داروها بود.
